Your Uganda Passport is the property of the Uganda government. It must be kept in a safe, secure place. It is advisable that you note the passport number and its date of issue on a separate piece of paper and keep this information in a different place. In the unfortunate event of its getting lost, it is your obligation to report the loss to the local authorities, without delay. Remember, should any unauthorised person use your passport fraudulently, or for perpetrating a felony, you are liable. Report the loss of it to your local police and obtain a police report.
The passport replacement fee is US$180, payable to "Uganda Embassy" by money order or cashier's cheque. Personal cheques will not be accepted. If you need an Emergency Travel Certificate, there will be an additional fee of US$50.
In an effort to expedite your business transaction with the Embassy, and save you valuable time, we have placed the necessary form (s) on the Internet for you to download at your convenience.
This is what you will need to remit:
1. A Police Incident Report documenting the loss of your passport.
2. A written (word processed) explanation of the circumstances
leading to the loss of your
passport.
3. A filled in and notarised AFFIDAVIT For Lost Passport
form.
4. A filled in Form A: APPLICATION FOR A UGANDA
TRAVEL
DOCUMENT.
5. A completed Form I: PARTICULARS OF A LOST
PASSPORT
QUESTIONNAIRE.
6. A filled in DECLARATION FOR UGANDANS
ABROAD form.
7. A filled in CITIZENSHIP VERIFICATION
form.
8. A filled in APPLICATION FOR AN EMERGENCY
TRAVEL CERTIFICATE form.
9. A fee of
US$180, by money order or cashier's cheque for a new Passport to be
processed.
10. An additional fee of US$50 for issuance of an Emergency Travel Certificate.
11. The passport number or file number of the lost document.
12. Two (2) passport size photographs; regular not digital, colour or black/white. The
photograph must show your full face; no head gear or hats of any kind. Neatly print/write your full name on the backside of one of the photographs.
13. STUDENTS ONLY: A letter from a school official, on official headed paper, certifying that you are a bona fide student at that school.
14. Enclose all the items and a money order or cashier's cheque for the processing fee(s) in an envelope. Please include a self-addressed, return pre-paid U.S. Postal Express envelope for shipping your document(s) back to you.
